Overview
Dependent eligibility for Modern Health is defined by your employer. Adult dependents can self-register using the primary benefit holder's details. For minor dependents, add them to Family Care via the Modern Health app and connect them with a therapist. For more information, check with your Benefits Team or visit the provided links.
Q: Is my dependent eligible for Modern Health?
A: Modern Health defines dependent eligibility based on how your employer defines “dependent.” Dependent eligibility and coverage will vary based on your employer's care plan with Modern Health. If you are unsure if you have dependent coverage or how your company defines dependents, please check with your Benefits Team.
Q: How can my adult dependents sign up?
A: If your dependent is an adult (aged 18 or older), they are able to register without an invitation from you, the primary benefit holder. Please kindly note that they will need your details as the primary benefit holder to register. To begin the self-registration process, they can visit this page and select the "For Family Members" icon located on the second half of the page. You can find more information here.
Q: How can my minor dependents sign up?
A: If your benefit plan includes minor dependent coverage, you can start the process of connecting your dependent with a therapist by adding them to your Family Care via the Modern Health web or mobile app (iOS / Android). Here, you’ll be asked to specify your dependent’s care needs. After you fill out the questionnaire for your minor dependent, you will be able to connect your dependent with a provider whose expertise aligns with their needs. Once you connect with a therapist who’s a great match for your dependent, you can schedule your dependent’s therapy sessions. You can find more information here.
