Overview
With the new Electronic Medical Record (EMR) experience, you may be asked to sign consent forms and complete your intake form directly in the Modern Health app. Your intake helps your provider understand your needs and prepare for care. If your consent form isn’t completed before your first therapy session, the session may not be able to proceed. If you don’t complete the intake form, your provider may devote time in your first session to review and complete this form with you.
What’s New
If you see the consent and intake forms within Modern Health, you’ll need to complete the required consent and intake paperwork in the Modern Health app before starting therapy. Your provider may send you other pre-session paperwork outside of Modern Health instead; if so, please fill that out.
When You Receive Your Consent & Intake Forms
You may be prompted to complete your consent and intake form after you’re matched with a therapist and before your first session begins.
How to Complete Your Consent and Intake Forms
Web App
- After you book your initial evaluation with a new provider, if your provider uses the Modern Health intake and consent forms, you will be prompted to complete them right away after booking your session.
- Open the Modern Health web app and follow the prompt on your home screen to complete your intake.
- You will also receive emails to complete incomplete pre-session paperwork as needed.
- Review and sign the consent forms, then complete the intake questions.
- Click Submit when finished. You can message your provider if you need to clarify something later. Your provider will likely go into some of the details of your intake form live with you in your first session as they get to know you.
Mobile App

What Information Do We Collect and Why?
Our therapists will ask you to fill out intake paperwork and consent forms through the Modern Health platform before your first session. Some therapists may use a specific software/portal outside the Modern Health platform to share the necessary forms before your upcoming sessions, while others will send forms within the Modern Health app. Please kindly note that these forms are standard practice for therapy.
The information we collect includes:
- Your consent to conduct therapy
- Mental health care needs and goals for therapy
- Medical History
- Demographics
If You Don’t Complete Your Forms
- Your first therapy session may be delayed or canceled if your consent form is not completed.
- If your intake form is not completed, your provider may, instead, devote time during your initial session to completing your intake form together.
How Your Information Is Used and Who Can See It
- Your intake and consent information is used to prepare for and deliver your care, consistent with our HIPAA Notice of Privacy Practices.
- Your matched provider uses your responses on your intake form to plan your first session and ongoing care. Messaging with your provider is available in‑app for clarifications or updates.

FAQs
Why do I need to fill out an intake form?
Intake Forms are used by providers in the mental health and medical fields to help your provider understand your clinical needs, set goals, and prepare for your first session.
Can I edit my intake form after submitting?
If anything changes after you submit, message your provider from the Messages tab to share updates, or contact Member Support for help.
Who can see my notes and forms?
Your information is used to deliver your care and handled according to our HIPAA Notice of Privacy Practices. Review our Data Privacy article for how your data is protected and your rights (including access or deletion requests).
What if I switch providers?
If you change providers, your new provider will have access to the intake form you originally filled out. If you want to add any context not addressed in your first intake form, please message your provider directly to share updates.
How do I withdraw consent?
To withdraw your consent for all therapy services provided through Modern Health, please email help@modernhealth.com and state your intent to remove your consent for therapy. A Member Support team member will respond within 2 business days and assist you with your withdrawal of consent for therapy services. This will remove your ability to access your therapy benefit provided by Modern Health. If you change your mind after submitting a withdrawal of consent for therapy, when you go to match with a new therapist, you will be prompted to complete a new consent form.
