Overview
If it is included in your benefit plan, Modern Health allows you to invite dependents to access care. Dependents are defined based on the employer's definition. For dependents 18 or older, invitations can be sent through the app or dependents can join through self-registration. Minor dependents (under 18-years-old), have an alternate flow outlined in this article. After eligibility confirmation, dependents will receive a unique registration link and their own allotment of sessions once registered.
Note: Modern Health defines dependent eligibility based on how your employer defines “dependent.” Dependent eligibility and coverage will vary based on your employer's care plan with Modern Health. If you are unsure if you have dependent coverage or how your company defines dependents, please check with your Benefits Team or refer to your company's FAQ document.
How to Join as a Dependent
If you are an adult dependent (aged 18 or older), you are able to register without an invitation from the primary benefit holder. Please kindly note that you will need the details of the primary benefit holder to register. To begin the self-registration process, please visit this page and select the "For Family Members" icon located on the second half of the page.
- When you visit the Modern Health website, click Sign up.
- On the next page, you will see Let's get you signed up. At the bottom of the page, please click "For family members (ages 18+)"
- Under Company name or code, please put your parent/spouse/partner's company name. For instance, if the primary benefit holder works at 'Modern Health,' please put 'Modern Health'.
- On the next page, please fill out the form and create your account!
How to Invite a Dependent
If your dependent is 18 years or older, you can invite them to use the Modern Health platform by logging into the Modern Health web or mobile (iOS / Android) app.
If your benefit plan includes dependent coverage, you can invite your dependent to register and request services via the Modern Health If your dependent is a minor (under 18-years-old), please follow the steps listed in this article.
Note: After following the below steps, your dependent will receive an email that contains a unique registration link. Your dependent must follow the unique registration invitation link to register successfully. Once registered, your dependent will receive a separate individual benefit allotment. Dependents can then expect to go through their first well-being check-in and initial intake to be provided a personalized wellness care plan.
Web App
- (Skip this step if you have already taken the onboarding assessment) Log in and select Get care for your family. Move to step 3 after this selection.
- Select your name at the top right-hand corner, then click Invite Dependent
- Based on your dependent’s age, click Learn more within the category your dependent falls under.
- Review your dependent’s benefits. If you would like to submit a dependent invitation, click Get started.
- Complete and submit the dependent eligibility request form.
- After confirming your dependent's eligibility, click Invite to join.
- Once you have entered and confirmed your dependent’s information, click Send invite.
- You may return to your Family Care page to monitor the progress of your request.
Mobile App

(Skip this step if you have already taken the onboarding assessment) Log in and select Get care for your family. You may have to scroll down to view this option.
- Based on your dependent’s age, tap the category your dependent falls under.

Review your dependent’s benefits. If you would like to submit a dependent invitation, tap Get started.

Complete and submit the dependent eligibility request form.

After confirming your dependent's eligibility, tap Invite to join.
- Once you have entered and confirmed your dependent’s information, tap Send invite.

You may return to your Family Care page to monitor the progress of your request.
Troubleshooting Steps
Experiencing trouble with inviting your dependent? Please note that in order to proceed, you must select the country from the drop-down menu under "Where are they located?" If you manually type in the country, copy and paste it, or if your web/mobile app autofills the field, the State and Zip Code fields may not populate correctly.
Once you select the country from the drop-down, the State and/or Zip Code fields should automatically populate. After filling out these fields, you should be able to click 'Continue' to proceed.
