Overview
You may change and remove your dependent's provider by using the Modern Health website or app. If you notice that your dependent’s therapist(s) are not surfacing on their Care Team in the Modern Health app, please submit a request at the top right of the browser and our Care Team will assist you.
Instructions
Web App
To change or remove a provider in the web app, please follow these steps:
1. Log into the Modern Health app.
2. From the home screen, under Your 1:1 Care, click View family (next to Manage care for your family).
3. Tap the ellipsis icon next to your chosen provider and, depending on your preference, tap Change provider or Remove from care team.
Mobile App
To change or remove a provider in the mobile app, please follow these steps:
1. Log into the Modern Health app.
2. From the home screen, under Your 1:1 Care, tap Manage care for your family.
3. Tap the ellipsis icon next to your chosen provider under your dependent’s Care Team. Depending on your preference, tap Change provider or Remove from care team.
Note: If you notice that your dependent’s therapist(s) are not surfacing on their Care Team in the Modern Health app, one reason may be that you are located in a country where in-app therapy is not currently supported. In this case, while therapy sessions may still be covered by your benefit, your sessions will take place outside of the app using your provider's personal video link. If you are interested in getting your dependent matched to a new provider, please submit a request at the top right of the browser and our Care Team will assist you.
