If your provider is part of our network, please submit a request so our Care Team can match you and you can utilize your Modern Health benefit. Please kindly note, if your provider is not a part of our network then our team would be happy to match you with a different provider so you can utilize your Modern Health sponsored sessions.
Note: Your Modern Health benefits must be used with one of our in-network providers and we do not offer reimbursement for sessions with providers not in our network.
