Working With Your Current Provider

Overview

If you are currently working with a provider, please reach out to them to confirm if they’re part of the Modern Health network. If you confirm your existing provider is part of our network, please submit a request so our Care Team can match you and you can utilize your Modern Health benefit.

Note: Your Modern Health benefits must be used with one of our in-network providers and we do not offer reimbursement for sessions with providers not in our network.
