Modern Health cannot guarantee the provision of official documentation for disabilities or accommodations. Some therapists and medical practitioners may be able to provide such documentation, including FMLA forms, work accommodation documentation, and support letters for Emotional Support Animals. If needed, please contact your therapy provider or submit a request for assistance from the Care Team.
Requesting Documents
Modern Health cannot guarantee that you can obtain official documentation, forms, or letters. Some, but not all, therapists, counselors, and medical practitioners (such as medical doctors and nurse practitioners) are able to assess for and complete this kind of documentation, which may include:
- FMLA (forms for leaves of absence related to the Family Medical Leave Act
- Medical documentation for work accommodations
- Letters in support of someone having an Emotional Support Animal present in specific settings (housing, travel)
If you are in need of this kind of documentation, we recommend that you reach out to your therapy provider or medical team for further discussion and assistance. If your current provider is unable to support your need, please submit a request at the top right of the browser and our Care Team will do our best to assist you (for example, we may be able to locate a therapist who can assess whether you meet the criteria for the requested documentation).
