Modern Health is dedicated to safeguarding your data privacy. Our Privacy Notice outlines the types of data collected, usage, retention practices, and protection measures. Deleting Your Modern Health Account and Data

Modern Health is committed to protecting the privacy of your data. Please visit www.modernhealth.com/privacy-notice for detailed information about what data we collect, how we use that data, our retention practices, and the steps we take to protect your privacy.

By requesting a data deletion, you are confirming that you would like Modern Health to delete your Modern Health account and the personal information associated with it. Please note, by deleting your account and associated data you will no longer be able to access the Modern Health platform moving forward.

To request a data deletion, you can follow these steps:

1. Fill out this form to start the process of deleting your account and data. Please use the email associated with your Modern Health account. This form is also available within the Modern Health mobile app in the account settings under “Delete Account”.
2. Once you click Make a Privacy Request, select "Member"
3. Select the action you want to take. Data Request or Delete all my data
4. Enter your email and submit it to verify your identity
5. You will receive an email to the email you used to verify your identity. Please note the link to verify will expire in 1 day.
6. Once you click the Confirm Email link, Fill in your details and submit
7. Your request will be submitted and you will receive an email receipt that your request is being processed. You can expect your account and data to be deleted within 30 days. Optional: You may choose to create an account in our DSR Request Center to track the status of and manage your request.
8. Once your request is completed, you will receive an email confirming that your data and account have been deleted.

Note: If you are still eligible for the Modern Health benefit through your employer, Modern Health will continue to receive your eligibility information contained within their eligible persons file. If you would like to be removed from future files sent to Modern Health please contact your employer. For any further questions, you can reach out to us at help@modernhealth.com.
